Percentages below are calculated over hospitals we have actually scanned, never over every hospital we track. Where no published file location is on record for a hospital, we report that as our own coverage gap and exclude it entirely, rather than counting it against the hospital.

Coverage gap: 4,406 tracked hospitals have not been scanned yet, because we do not have a confirmed published file location for them. They are excluded from every figure on this page. This is a limit of our crawl, not a statement about those hospitals.

We reached a published file for 226 more hospitals but could not finish evaluating it, because the host refused our crawler, rate-limited us, or the file exceeded what our scan could process. These are limits of our crawl. They are not findings about those hospitals, and they are excluded from every percentage above.
